[edit] How do I apply for an account?

Go to the main grid page for your campus(https://grid.ucla.edu or https://grid.ucsb.edu) and click on the Apply link in the upper left. There you must enter a valid login username/pw for a cluster on the pull down list. Fill out the rest of the form, and choose your cluster at the 'select a resource' at the bottom.

[edit] What if my usernames are different on the various clusters

This isn't a problem, you just choose a Grid username you want, and it will be automatically mapped to your various other usernames.
